Position: Geriatric Care Manager

Primary Purpose of Job

The primary purpose of this job is to provide Geriatric Care Management services to stabilize and improve quality of life for seniors living within the St. Joseph Parish boundaries in Downingtown, Bucks County.This job responds to the evolving needs of aging clients through in-home social service provision.The position requires on-the-spot decision-making and high level of independent judgment. Coordination with client's family members, professionals and organizations, and other resources is essential. This position is a 16 hour per week position.             

Essential Job Functions

  • Perform assessments to determine strengths and barriers to healthy aging.
  • Develop Care Plan with the client to set goals and identify corresponding interventions and resources.
  • Connect clients to appropriate services that enhance their quality of life.
  • Assist the client in navigating through process to access services and advocate for clients to overcome barriers to services and supports.
  • Provide counseling and supportive services as needed by the clients.
  • Conduct home visits and follow-up telephone calls to provide assessment, care planning, and supportive services as needed in-home, carrying a caseload of clients in the Downingtown area.
  • Document interventions in the electronic file and provide written and verbal communication with clients.
  • Perform outreach to engage potential clients in the program.
  • Coordinate with other members of the parish staff.
  • Keep abreast of changes in client needs to proactively initiate any changes in level of care.
  • Maintain the confidentiality of all client care information.
  • Maintain statistics on social work interventions and compile reports to CHCS as required.
  • Perform other duties as assigned by the supervisor.

  • Responsible to be knowledgeable and to implement the Mission and Philosophy of the organization within the scope of responsibility.
  • Responsible for overall client satisfaction as it relates to the Eldercare Program Manager duties.
  • Treat all staff, clients, volunteers, and visitors with dignity and respect.
  • Maintain a good working rapport with interdepartmental personnel, Archdiocesan partners, clients and their families.




  • Master's degree from an accredited university in Social Work, Gerontology or related field required.


  • Minimum three (3) years' experiences working with seniors.
  • Knowledge of the local community, resources, federal and state benefits programs and their eligibility requirements.
  • Must have a current driver's license, vehicle insurance, reliable transportation and be able to meet the Authorized Driver criteria as required by the Archdiocese Office for Insurance Services.

Physical Requirements (With or without the aid of mechanical devices)

  • Sit, stand, bend, lift and move intermittently during work hours.
  • Must be able to lift, push, pull and move equipment, supplies, etc. up to 50 lbs.

Work Environment

  • Must function independently and have flexibility, personal integrity and the ability to work effectively with participants, personnel and other agencies.
  • Must be able to work with older adults in their homes respectful of their right to self-determination.
  • Must be comfortable marketing the eldercare program and conducting outreach to parishioners
  • Is subject to frequent interruptions.
  • Is involved with participants, personnel, visitors, government agencies/personnel, etc., under all circumstances and conditions.
