Position: Social Service Coordinator 

Primary Purpose of Job

The primary purpose of this position is to be responsible for providing social service intervention to the participants of the senior center and older adults from the community. 

Essential Job Functions

  • Provide brief services and information and referral assistance.
  • To assess and file eligibility documents for commodities boxes, and other entitlement programs.
  • Provide a weekly mental health/supportive group to participants.
  • Assist with filing applications for SNAP, LIHEAP, Rent/Tax rebates etc.
  • Maintain documentation and case files.
  • Register new participants, complete PA Screens and Nutrition Assessments in compliance with PCA center standards of operation.
  • Meet with supervisor and also attend center team meetings.
  • Maintain confidentiality of consumer information.
  • Attend training, meetings, and other in-service to complete and enhance job performance.

Other Job Functions

  • Responsible to implement the Mission and Philosophy of the organization within the scope of responsibility.
  • Work independently with a high degree of tact, discretion and professionalism.
  • Work collaboratively and cooperatively with Catholic Human Service, PCA, and other community groups.
  • Develop and maintain a good working rapport with interdepartmental personnel, other departments and partners of the program to assure that all services and activities can be adequately maintained to meet the needs of the participant.
  • Treat all staff, participants, residents and visitors with dignity and respect.
  • Submit data, consumer satisfaction surveys, and program evaluation as it relates to Quality Assurance and contractual guidelines.
  • Performs other duties as assigned by supervisor.



  • Bachelor of Social Work or related degree.


  • Current driver's license, vehicle insurance and/or reliable transportation
  • Experience working with older adults.
  • Bilingual in Polish and English preferred

Physical Requirements (With or without the aid of mechanical devices)

  • Sit, stand, bend, lift and move intermittently during work hours.
  • Must be able to lift, push, pull and move equipment, supplies, etc. up to 50 lbs.

Work Environment

  • Must function independently and have flexibility, personal integrity and the ability to work effectively with participants, personnel and other agencies.
  • Must be able to work with older adults in a community setting respectful of their right to self-determination.
  • Is subject to frequent interruptions.
  • Is involved with participants, personnel, visitors, government agencies/personnel, etc., under all circumstances and conditions.
