Catholic Housing & Community Services is currently seeking a part-time Social Service Care Manager (24 hours/week) for an affordable housing site located in Philadelphia, PA.


Primary Purpose of Job

The primary purpose of this job is to respond to the evolving needs of aging residents through in-home and office-based social service provision. The position requires on-the-spot decision-making and high level of independent judgment. Coordination with residents' family members, area professionals and organizations, and the property management team is essential. The Social Service Care Manager will effectively assess residents' current needs, aim to identify trends and anticipate future needs to develop appropriate programming and interventions.                                                                             

Essential Job Functions

  • Manage a caseload of of approximately 75 senior residents of an independent living apartment community.
  • Assess all new residents for each resident's key strengths and barriers to successful aging in place, including physical and behavioral health/functioning, health insurance, finances, safety, mobility/fall risk, transportation, nutrition/food security, legal issues, and social support.
  • Create service plan of interventions individualized to the needs identified during the assessment process.
  • Navigate residents through process to access services and advocate for residents to ensure any roadblocks to service are overcome.
  • Manage and deliver services to meet the needs identified in the care plan including documentation, regular written and verbal communication with residents, and coordination of community resources.
  • Provide counseling and supportive services as needed by the residents.
  • Maintain confidentiality of resident information in accord with confidentiality policy, professional standards and other regulations.
  • Respond to emergent/crisis resident situations as they arise and communicate such incidents to both Catholic Housing and Community Services and property management.
  • Advocate for residents' needs with property management team and owner.
  • Act as onsite representative of owner as directed by supervisor.
  • Create a community atmosphere that addresses social, cultural, educational and spiritual needs of the residents' through in-house events and coordination with outside entities.
  • Maintain statistics on social work interventions and group participation.
  • Be accountable to Catholic Housing and Community Services, Pennsylvania Housing Finance Agency, and any other funding sources or stakeholders through written documentation and reports.
  • Adhere to established policies and procedures for Catholic Housing and Community Services at all times.
  • Demonstrate high standards of attendance and reports to work on time.
  • Perform all other duties as assigned by supervisor.


Other Job Functions

  • Responsible to be knowledgeable and to implement the Mission and Philosophy of the organization within the scope of responsibility.
  • Responsible for overall resident satisfaction as it relates to the Social Services Coordinator Senior Housing duties.
  • Treat all staff, residents, volunteers, and visitors with dignity and respect.
  • Maintain a good working rapport with interdepartmental personnel, Archdiocesan partners, residents and their families.




  • Master's degree from an accredited university in Social Work, Gerontology or related field required.



  • Minimum three (3) years' experiences working with seniors.
  • Knowledge of federal and state benefits programs and their eligibility requirements.
  • Must have a current driver's license, vehicle insurance, reliable transportation and be able to meet the Authorized Driver criteria as required by the Archdiocese Office for Insurance Services.


Physical Requirements (With or without the aid of mechanical devices)

  • Sit, stand, bend, lift and move intermittently during work hours.
  • Must be able to lift, push, pull and move equipment, supplies, etc. up to 50 lbs.


Work Environment

  • Must function independently and have flexibility, personal integrity and the ability to work effectively with participants, personnel and other agencies.
  • Must be able to work with older adults in a community setting respectful of their right to self-determination.
  • Is subject to frequent interruptions.
  • Is involved with participants, personnel, visitors, government agencies/personnel, etc., under all circumstances and conditions.
